To reset your password, click "Forgot password?" on the log in page. Enter the email you used to create your account. You will receive an email with a link to reset your password.

When you log into your account, you will see a list of all of the courses you have purchased. To access a course, simply click on the name of the course in the list.

Five Conversations is designed primarily for people over the age of 50 who are ready to create a long-term plan for their care and wellbeing as they age. It may also be helpful for friends and family members who are helping a loved one create a plan.  

If you think Five Conversations may be helpful for you, we encourage you to take the course with friends and family who are in a similar phase of life, or who may be supporting you in your planning journey. We find that planning with community can be a more supportive and meaningful experience.

Long-term care is a term used to describe home- and community-based services to support health and wellbeing as we age. It’s important to create your long-term care plan early, so you and your loved ones are able to thoughtfully prepare for your future.

Through this course, you will create a comprehensive plan for long-term care and wellbeing, with five important conversations about your finances, housing, health, and wellness.

In conversation #1, you will learn how to identify and invite your Care Circle—the trusted people who will offer support, companionship, and care in the coming years. 

In conversation #2, you will create your personal healthcare directive and identify who you would like to be your healthcare proxy.

In conversation #3, you will identify your health, wellness, and home care needs to get a better understanding of what kind of support you may need in the future, and how to plan for it. 

In conversation #4, you will explore how and where you would like to live in the future based on your care needs, lifestyle preferences, and financial resources.

In conversation #5, you will contemplate your wishes and values to prepare for end of life with dignity.

This is an important experience to share with loved ones. You may find it helpful to print and share the course worksheets, or to schedule time to watch the videos with family and friends. 

We also encourage you to take this course with others who are in the same phase of life as you so you can support each other in this journey. If a friend or family member would like to take the course along with you, they can purchase the course at a 25% discount. When you register for the course, we’ll email you a discount code to share with family and friends.

We offer an online gathering every few months to discuss long-term care planning and share experiences with our community. This gathering is open to anyone who is enrolled in Five Conversations. You will receive an invitation at least a month in advance via email and the course platform. 

Please note that our founder, Sierra Campbell, is not accepting any new private clients at this time.

We know that planning for your future is an ongoing journey. So, when you purchase a Nurture course, you are always granted lifetime access. 

Please note that lifetime access only includes the course features and resources that are available at the time of purchase. It does not include future updates to the course.

You can complete the course at whatever pace feels comfortable for you—as slowly or quickly as you like. However, if at all possible, we recommend completing the course within 2 to 3 months. This will help you stay on track with completing your long-term care plan.
